What they aren’t telling you about Medicare
Medicare is a crucial program for seniors, but there are several hidden details that many people don’t know until they enroll. Here’s what they aren’t telling you about Medicare:
1. Medicare Is NOT Free
Many assume Medicare is free, but it isn’t—you still pay premiums, deductibles, and copays.
Part A (Hospital Insurance): Free for most, but costs up to $505/month (2024) if you didn’t pay enough Medicare taxes.
Part B (Medical Insurance): $174.70/month (2024) for most, but higher for high-income earners.
Part D (Prescription Drug Plans): Varies but can cost an additional $10–$100+/month depending on the plan.
2. Medicare Does NOT Cover Everything
Medicare has major gaps, and many seniors are shocked by the out-of-pocket costs. Medicare does NOT cover:
Dental Care (exams, cleanings, dentures, root canals)
Vision (glasses, contacts, routine eye exams)
Hearing Aids
Long-Term Care (Nursing homes & assisted living are NOT covered)
Routine Foot Care
Most Alternative Therapies (Acupuncture, chiropractic, etc.)
➡ Solution: You need Medicare Supplement (Medigap) or a Medicare Advantage plan to help with costs.
3. The “Free” Medicare Advantage Plans Aren’t Really Free
Medicare Advantage (Part C) plans advertise “$0 premium” plans, but they come with trade-offs:
Limited Provider Networks – You may not be able to see your current doctor.
High Out-of-Pocket Costs – Copays, deductibles, and co-insurance add up.
Prior Authorizations Required – You may need approval for treatments, delaying care.
➡ Solution: If you want more provider flexibility, consider Original Medicare with a Medigap plan.
4. Enrollment Mistakes Can Cost You—For Life
If you don’t sign up at the right time, you could face permanent penalties.
Medicare Part B Late Enrollment Penalty: 10% extra per year you delay (for life).
Medicare Part D Late Enrollment Penalty: 1% extra per month delayed (for life).
Medigap Enrollment Window: You have 6 months after turning 65 to get a Medigap plan without medical underwriting—after that, you can be denied or charged higher rates.
➡ Solution: Plan ahead and enroll on time to avoid lifelong penalties.
5. Medicare Drug Plans (Part D) Have a Coverage Gap (“Donut Hole”)
Once your total drug costs hit $5,030 (2024), you enter the donut hole.
While in the donut hole, you pay 25% of drug costs until you reach catastrophic coverage ($8,000 in out-of-pocket spending).
➡ Solution: Use generic drugs, look into Extra Help (low-income subsidy), and compare plans annually.
6. Medicare Won’t Cover You Overseas
Original Medicare does NOT cover medical care outside the U.S.
Some Medigap plans (Plan G, Plan F) cover up to 80% of emergency care abroad.
➡ Solution: If you travel, consider a Medigap plan or travel insurance.
7. You Might Get Hit with “IRMAA” If You Make Too Much
If you make over $103,000 (single) or $206,000 (married), you’ll pay higher Part B and Part D premiums.
This extra fee is called Income-Related Monthly Adjustment Amount (IRMAA).
➡ Solution: Lower your taxable income before retirement (use Roth IRAs, tax-free withdrawals).
8. Medicare Won’t Automatically Cover Nursing Home Care
Medicare only covers short-term rehab stays (up to 100 days) after a hospital stay of at least 3 days.
It does NOT pay for long-term nursing home stays or assisted living.
➡ Solution: Consider Long-Term Care Insurance or Medicaid planning.
9. You Must Compare Plans EVERY YEAR
Medicare plans change annually—what worked last year might be more expensive next year.
Doctors, drug coverage, and costs can change each year.
Open Enrollment (Oct 15 – Dec 7) is the only time to switch plans.
➡ Solution: Review your plan annually to avoid surprise costs.
Final Thoughts: Be Informed, Be Prepared
Medicare is complex, and not knowing these details can cost you thousands. Plan ahead, compare options, and ask questions.
